CAP Pilot Training Checklist
__________ Take Level 1 course (see schedule or ask program officer for class time)
__________ Make a copy of your FAA Pilot's License
__________ Make a copy of your current FAA Flight Physical
__________ Make a copy of your current CAP Membership Card
__________ Make a copy of the last page of your logbook showing total hours
__________ Make a copy of the last BFR recorded in your logbook
__________ Complete a Statement of Understanding (CAPR 60-1, Attachment 3)
__________ Obtain and complete a Pilot Data Summary Form (NMWF-15)
__________ Submit NMWF-15 and above mentioned copies to Operations Officer for processing
__________ Receive a New Mexico Wing Aeronautical Identification Card (blue card)
__________ Thoroughly read CAP Flight Management Regulations (CAPR 60-1)
__________ Familiarize yourself with the Aircraft Pilot Operating Handbook
__________ Complete an Aircraft Questionnaire (CAPR 60-1 Attachment 2)
__________ Complete Form 5 Written Examination (Select CAPF-5 Exam Online) (CAP Online Forms)
__________ Take a Form 5 checkride from a squadron check pilot
__________ Present your "blue card" and logbook to the check pilot for endorsement
